Brain sheets are not a replacement for charting, orders, handoff policy, or the medical record. They are personal shift organization tools that help you keep report, priorities, reminders, and reassessments in one place while you work.

How to Use Brain Sheets Safely

  1. Use the sheet to organize your thinking, not as the official chart.
  2. Follow your facility rules for patient identifiers, printed materials, and end-of-shift disposal.
  3. Do not leave sheets in public areas, break rooms, elevators, or unsecured bags.
  4. Confirm medication times, orders, allergies, code status, and clinical changes in the approved charting system.
  5. Shred or dispose of any paper containing protected patient information according to policy.
Nursing safety note: This tool is for education and shift organization only. Always follow your facility policy, provider orders, medication administration rights, and current clinical guidelines.
